The cadet section of Helston St John Ambulance may be forced to close if a new leader cannot be found.
There is no shortage of young recruits signing up for the cadets - a waiting list has recently been started - but finding a long-term voluntary leader is proving more problematic.
Helston divisional superintendent Sheila Hilditch said: "We haven't suspended it yet but we might have to close down the cadet bit if we can't find someone to take over.
"It would be a real shame, not only for the cadets themselves, but also for the Badgers who are due to be moving up into the cadets."
The cadet leader works with youngsters aged from ten to 18 and has the opportunity to get involved with first aid, as well as personal development and leadership courses.
Training is provided and no specific qualifications are required.
