Harlain Mbayo scored the winning goal in the second half as Truro regained the trophy, which is contested by the two sides every summer.
Rio Garside gave Truro a half-time lead before Jack Bowyer levelled for Falmouth after the break, only for Mbayo to head in the winning goal two minutes later.
